How does Refluxora work?

Is Refluxora a medical device?

No. Refluxora is a Health & Fitness wellness companion, not a medical device. It does not diagnose, treat, cure, or prevent any condition. Always consult your doctor, gastroenterologist, otolaryngologist, or OB-GYN before starting, changing, or stopping any treatment. See the Medical Disclaimer.

Can Refluxora tell me if I have GERD or LPR?

No. Refluxora cannot diagnose GERD, LPR, Barrett's esophagus, esophagitis, hiatal hernia, eosinophilic esophagitis, or any condition. It explains what symptoms and patterns may mean against published guidance and helps you track them — but a diagnosis can only come from a qualified doctor or ENT who sees your full clinical picture, including endoscopy and pH monitoring where indicated.

What is the RSI score and what does ≥13 mean?

The Reflux Symptom Index (RSI) is a validated 9-item self-report questionnaire developed by Belafsky, Postma, and Koufman (2002) for laryngopharyngeal reflux screening. A total score of 13 or above is associated in the literature with increased likelihood of LPR. RSI is a screening tool, not a diagnostic test. A high RSI score means "discuss with your ENT" — it does not confirm LPR. Refluxora tracks RSI weekly so you can see the trend over time and bring it to your ENT visit.

How does Menu Pre-Scan work?

Open Refluxora at a restaurant. Tap the 3rd card on the Today screen ("Heading out?") or the Menu chip in +Log. Photograph the menu page. Refluxora uses on-device Apple Vision to read dish names, then ranks them per your cohort: refractory triggers (Brian), OB safety + bedtime guards (Patricia), flavor-preserving swaps (Frank), drug interactions (Sam), Acid Watcher Phase compliance (Anna), or pH and Acid Watcher tagging (Larry). You see top picks (mint), worth-it (gold), and skip (alert) — usually within 2 seconds. Optional: tap a dish to discuss with the AI Coach, or tap "I'll order this" to log it for later correlation.

How does the Meal Scan work?

Snap a meal and Refluxora returns an educational reflux-trigger rating — safe / watch / alert — with a 0–10 trigger score based on general dietary guidance for GERD and LPR (ACG 2022 lifestyle pillars, IFFGD, Mayo, and Dr. Aviv Acid Watcher protocols). It is not a personalized meal plan or clinical nutrition therapy. Meal photos are stored locally in your meal log.

My menu / meal photo has restaurant or personal info. Is that safe?

Menu and meal photos are processed on-device by Apple Vision first. They are sent to OpenAI's vision model ONLY as a fallback when on-device confidence is low — and only after you accept the per-feature consent. The image is sent over an encrypted connection only for OCR / rating and is not retained by the AI provider after processing. Refluxora does not store photos on any server of ours. We do not extract or store restaurant names, addresses, or any venue identification. See Privacy Policy §1.2 and §1.3.

What does the AI Reflux Coach actually know?

The Coach answers from an on-device knowledge base of ~2,400 chunks across published guidelines and peer-reviewed extracts: ACG 2022 GERD guideline, AGA 2024 Best Practice, AAO-HNS LPR position paper, Mayo Clinic and Cleveland Clinic patient material, Dr. Koufman and Dr. Aviv Acid Watcher protocols, the Voquezna (vonoprazan) FDA label, ACOG pregnancy reflux guidance, IFFGD lifestyle, AGS Beers Criteria, and selected PubMed literature on GERD, LPR, refractory reflux, and PPI taper protocols. Every reply includes a tap-able citation. If the knowledge base has no grounding for your question, the Coach says so rather than guessing.

Can the AI Coach diagnose me or change my PPI / Voquezna dose?

No. The Coach explains what guidelines say, but it is designed to decline diagnosis, medication-dosing, dosage modification, Barrett's prediction, and emergency questions and to redirect you to a clinician (GI, ENT, OB-GYN, or primary-care). Never start, stop, or change a prescribed medication based on the App. Always confirm with your prescriber.

I'm pregnant — is Refluxora safe to use?

Yes — Refluxora's Pregnant Patricia cohort is built specifically for pregnancy reflux. It references ACOG and AAP guidance, tracks Tums (calcium carbonate) against the daily calcium ceiling, surfaces FDA Pregnancy and Lactation Labeling Rule (PLLR) summaries, and the Menu Pre-Scan ranking flags caffeine and late-night risk. However: Refluxora is reference material, not OB advice. Sudden, severe, or worsening upper-abdominal pain — especially with vision changes, severe headache, swelling, or right-upper-quadrant pain — requires immediate OB evaluation (possible HELLP syndrome). Always follow your OB-GYN's guidance. See Medical Disclaimer §8.

I'm on Voquezna (vonoprazan) — what does the app do for me?

For the Burning Brian cohort, Refluxora tracks your Voquezna trial week-by-week with a symptom-severity overlay so you can see whether the medication is reducing your reflux burden over time. It also generates a doctor-shareable PDF formatted for your GI that includes your trigger pattern, RSI/symptom trend, and Voquezna efficacy timeline. Refluxora does not modify Voquezna doses, does not recommend continuation or discontinuation, and does not predict efficacy. Decisions about Voquezna belong to you and your GI.

I'm trying to taper off my PPI — can Refluxora help?

For the Anti-Pharma Anna cohort, Refluxora surfaces a general 12-week taper framework based on published taper guidance (ACG taper considerations, IFFGD lifestyle, and DGL / zinc-carnosine / alginate timing references) with rebound-flare watch alerts. The framework is educational, not a personalized taper plan. PPI tapering can cause rebound acid hypersecretion that may be uncomfortable or trigger new symptoms. Any taper plan must be discussed with and supervised by your prescriber. See Medical Disclaimer §4.

I take 7 medications — will Refluxora flag interactions?

For the Senior Sam cohort, Refluxora surfaces drug-interaction reference flags based on AGS Beers Criteria and published references (for example grapefruit with calcium-channel blockers like amlodipine; caffeine with stimulants; common PPI interactions). The flags are educational reference, not a clinical decision-support system. Always confirm interactions with your pharmacist. Refluxora does not adjust doses, recommend medications, or replace your pharmacist's review.

Does Refluxora connect to Apple Health?

With your permission, Refluxora can read selected Apple Health data (sleep position and duration, dietary water intake, body mass) to enrich sleep-position + meal-timing correlation. The access is read-only — the App never writes to Apple Health — and the data stays on your device. You can grant or revoke it in iOS Settings → Privacy & Security → Health.

Is my data private?

Yes. Reflux symptoms, RSI scores, photos, medication and protocol logs, symptom notes, and chat history are stored locally on your device. Photos and Coach messages are sent to AI providers (OpenAI Vision OCR fallback; DeepSeek V3 for Coach) only when you tap to use those features and after you accept the per-feature consent — they are not retained after processing. We do not collect your name, email, restaurant names, or location, and we do not use advertising or cross-app tracking SDKs. See the full Privacy Policy.

Are my photos used for face recognition?

No. The Meal Scan looks at food and the Menu Pre-Scan looks at a paper document — neither performs face recognition, biometric template extraction, or any facial analytics.

What does Refluxora cost?

Weekly $4.99 / week with a 7-day free trial; Annual $39.99 / year (≈ $0.77 / week) with a 7-day free trial. Both tiers grant identical access. Additional tiers — Annual Family and Lifetime — and optional add-ons (LPR Pro, Voquezna Companion, PPI Taper Pro, Diet Lab) are planned for a future release. Prices may vary by region.

How do I cancel my subscription?

iOS Settings → Apple ID → Subscriptions → Refluxora → Cancel Subscription. Access continues to the end of the current billing period. Refunds are handled by Apple at reportaproblem.apple.com.

How do I restore my purchase?

If you reinstalled Refluxora or switched devices: open Settings inside the App → Restore Purchases. Make sure you are signed in to the same Apple ID you used originally. RevenueCat will verify your entitlement with Apple.

Can I export my data for my doctor / ENT / OB?

Yes. Refluxora generates a doctor-shareable PDF summarising your RSI trend, symptom pattern, meal/menu picks, medication and taper log, and Acid Watcher / pregnancy / drug-interaction notes — with guideline-grounded citations. The PDF auto-formats for GI, ENT, OB, or primary-care templates depending on your cohort. Share it directly from the iOS share sheet.

How do I delete my data?

Because your data is stored locally, you can delete individual entries within the App, use Settings → Delete all my data to wipe every log at once, or uninstall the App. No request to the developer is needed — we do not hold your health data on external servers.

I found a bug. What do you need?

Email sergejkar@gmail.com with: (1) your iPhone model and iOS version, (2) the Refluxora build number (Settings → bottom of page), (3) what you tapped right before the bug, (4) which cohort you selected during onboarding, (5) a screenshot if it is visual. We respond within 24 hours.

I think something is seriously wrong with my reflux right now — what do I do?

Refluxora tracks chronic patterns, not acute emergencies. Chest pain (especially if pressure-like or with arm/jaw/back radiation), difficulty breathing, vomiting blood, black tarry stools, food impaction, or progressive difficulty swallowing are an urgent-care or ER situation. For LPR red flags (hoarseness >3 weeks, painless neck mass, unintentional weight loss), seek ENT evaluation promptly. See the Medical Disclaimer §9 and §10 for full red-flag lists and crisis lines.
